一、只有 2025 年 11 月版的 IMM 5476 有效。
IRCC 于 2025 年 11 月发布新版 IMM 5476，旧版本的宽限期到 2026-03-12 为止；2026-03-13 起只收 11-2025 版。手上存的老 PDF 直接扔掉，重新下载。

二、ICCRC 这个名字已经没有了，现在叫 CICC。
本帖多处提到的 ICCRC（Immigration Consultants of Canada Regulatory Council，加拿大移民顾问监管委员会） 已被 CICC（College of Immigration and Citizenship Consultants，加拿大移民与公民顾问学院） 取代。这不是简单改名——监管机构从行业协会升格为法定学院，投诉与纪律机制随之改变。表格第 6 题要填的会员号，填的是 CICC 会员号。
现行的"授权代理"三类（与本帖当年的三类一一对应，只是第一类换了监管机构）：
- 移民/公民顾问 → 必须是 CICC 在册且信誉良好的会员；
- 律师或公证人 → 必须是加拿大某省/地区律师协会，或魁北克公证人协会（Chambre des notaires du Québec）的在册会员；
- 持牌法律助理（paralegal，仅安大略省） → 必须是安大略律师协会（Law Society of Ontario）会员。
三、CIC 已更名 IRCC（2015 年底），本帖里所有 cic.gc.ca 链接均已并入 canada.ca 并大多改版。
还有一条本帖没写、今天很要紧的边界：IMM 5476 不适用于护照申请。它管的是《移民与难民保护法》和《公民法》项下的申请与程序，护照业务另有一套授权规则。有人拿这张表去办护照，一定被退。

递交方式也变了：本帖写的"随申请一起邮寄"或"用 Case Specific Enquiry 网页表上传"，今天的主流是在 IRCC 在线账户里提交——线上申请会在流程中直接要你上传这张表。注意一个常见误解：顾问用 CICC 会员身份走"授权付费代表门户（Authorized Paid Representative Portal）"代交，并不能替代这张表。 门户只是递交通道，代理关系仍然要靠一份申请人与代理双方签字的 IMM 5476 来建立；IRCC 官网原话是"you must have them sign the use of representative form and submit it with your application"，并且特意加了一句"If your representative says you don’t have to give us their information, they’re wrong"。未签字的表按未完成表格退件，表上日期距 IRCC 收到超过 90 天（stale-dated）或填了未来日期的，同样退件。增删改代理的现行说明 → https://www.canada.ca/en/immigration-refugees-citizenship/services/immigration-citizenship-representative/change-cancel.html
这张表的核心逻辑，十年没变，也是本帖最值钱的部分：
签这张表你同时做了两件事——授权某人代你与 IRCC/CBSA 打交道，以及授权 IRCC 把你的档案信息交给这个人。第二件事才是重点：签完之后，IRCC 的所有信函会发给代理，不发给你。你不再是第一时间知道消息的人。这就是为什么"随便找个朋友帮忙填一下"是个危险的决定——不是因为他不专业，是因为你的补料信、面试通知、拒签理由都会先到他手上。
同样十年未变的还有三条硬约束：一份申请同一时间只能有一个代理（指定新的，旧的自动失效）；收费代理必须是上述三类之一，否则整份申请退料；代理信息一变就要通知 IRCC。
下面是 2015 年原文，ICCRC 名称、CIC 名称、cic.gc.ca 链接与"邮寄至 CIC office"的递交方式均已过期（正文中已就地标注），表格分节结构（A/B/C/D 四节）与填写逻辑 2026 年仍然一致，保留作变更史对照。

2015年6月，加拿大进一步修改代理要求以配合最新更改的公民法，从2015年6月11日起，只有 CIC 认可的律师才能给移民入籍提供建议和代理。（2026 注：这句当年就写窄了，和本帖下方“授权代理定义”那条自相矛盾——授权代理从来不止律师一类。现行三类见文首第二条。）
代理(representative)是代表你与CIC或CBSA打交道的人(他们为你提供建议和指导并作为你和CIC之间的中介)，你不需要指定代理(do not need to)，如果指定别人代表你与CIC处理你的申请并且授权CIC将你的信息发给代理人则必须填写本5476表，一个申请同一时间点只能指定一个代理(have one representative only per application)。
You do not need to hire a representative, it is your choice. No one can guarantee the approval of your application. All the forms and information that you need to apply are available for free at www.cic.gc.ca.
A representative is someone who has provided advice or guidance to you at any stage of the application process, whether that person received consideration (i.e. compensation) or not.
Following the submission of your application, that person may conduct business on your behalf with Citizenship and Immigration Canada (CIC) and the Canada Border Services Agency (CBSA) if you appoint them as your representative by filling out this form. The preceding includes Express Entry submissions.
You may have one representative only per application. If you appoint an additional representative, the previous representative will no longer be authorized to conduct business on your behalf and receive information on your case file. Note: You must use this form to appoint a paid or unpaid representative to conduct business with CIC or the CBSA on your behalf. You must also use this form to:
- notify CIC if your representative’s contact information changes,
- if you wish to cancel the appointment of your current representative and represent yourself, or,
- if you wish to cancel the appointment of your current representative and appoint a new representative
指定代理后（APPOINTMENT OF REPRESENTATIVE）任何代理信息的修改都要及时通知CIC。

警告：申请表里面的任何不实信息都是严重的犯罪，你有权不使用代理，如果提交的这个5476表就必须保证里面的所有信息都真实可靠。申请人和律师都不能在5476表里有不实声明。

填写5476代理表最核心的目的是申请人授权所指定的代理与CIC直接打交道，同时书面同意CIC向代理披露你的信息，否则违反隐私法。如果你不希望将你的信息共享给其他人请谨慎填写该代理表。填写该申请表意味着你同意CIC将这些信息共享给代理人。5476表里面的代理人信息必须真实，如果里面的代理是不合格的将被退料。
同时，5476表上的信息都可能会与第三方机构共享，包括皇家警局。
The information you provide on this form is collected under the authority of the Immigration and Refugee Protection Act and the Citizenship Act and will be used in assessing your application according to the requirements of the Act. It will be retained in a Personal Information Bank identified in Infosource. The information may be shared with other organizations such as the Canada Border Services Agency (CBSA), the Royal Canadian Mounted Police (RCMP), the Canadian Security and Intelligence Service (CSIS), where there is an agreement or arrangement with a foreign government, in accordance with subsection 8(2) of the Privacy Act. Pursuant to the Immigration and Refugee Protection Regulations and the Citizenship Regulations, the information may also be shared with a regulatory body that is responsible for governing or investigating the conduct of representatives, such as a provincial and territorial law society, the Chambre des Notaires du Québec and the Immigration Consultants of Canada Regulatory Council (ICCRC). Under the Privacy Act and the Access to Information Act individuals have the right to protection of and access to their personal information. 5476代理表适合所有签证及移民部公民项目（团聚移民，技术移民，投资移民，工签，留学，旅游，访问，入籍），只要申请过程中申请人需要代理人与使馆直接联系的都需要填写代理表 IMM 5476 ，下面是详细的 IMM5476E代理表填写指南（Guide 5561）。（2026 注：护照申请除外，见文首。）
Use of a Representative (IMM 5476)
年龄18周岁以上，需要代理代表申请人处理一切事务的都需要填写完整的5476表，5476表的功能有3个：指定，更新或撤销代理人。
Who may use this form?
Complete this form only if you:
- are appointing a representative;
- have to update contact information for your previously appointed representative; or
- are cancelling a representative’s appointment.
If you have dependent children aged 18 years or older, they are required to complete their own copy of this form if a representative is also conducting business on their behalf.
代理定义：在申请过程中或诉讼过程中你指定的代表你与CIC或CBSA直接打交道的人，这个人会给你提供咨询，顾问和指导。
Who is a representative?
A representative is someone who provides advice, consultation, or guidance to you at any stage of the application process, or in a proceeding and, if you appoint him or her as your representative by filling out this form, has your permission to conduct business on your behalf with Citizenship and Immigration Canada (CIC) and the Canada Border Services Agency (CBSA).
是否使用代理由申请人自行决定，使馆会公平对待所有以自己名义递交的申请人和所有指定了代理的申请人，指定代理人不会加快处理速度，也不会给予特殊关照，这个代理人只能代表你与CIC处理你本次申请相关事项，任何申请阶段只能指定一个申请人，当你指定代理人时就需要提交5476表，授权CIC或CBSA将你的信息共享给该代理人。指定代理人后CIC将直接联系代理人而不是申请人。
You are not obliged to hire a representative. We treat everyone equally, whether they use the service of a representative or not.
When you appoint a representative:
- you also authorize CIC and CBSA to share information from your case file with this person in place of you. Please note the representative will receive all correspondence from CIC or the CBSA, not the applicant;
- your application will not be given special attention nor can you expect faster processing or a more favourable outcome;
- the representative is authorized to represent you only on citizenship or immigration matters related to the application you submit with this form; and
- you can appoint only one (1) representative for each application you submit.
Important information: You must notify CIC if your representative’s contact information changes, or if you change your representative, or cancel the appointment of your representative. 代理分免费和收费2种：免费代表你（家人朋友，临时帮助或特殊情况类型），收费做你的代理（授权律师，专业做代理的）。
亲戚朋友，某些组织，律师等都可以做免费代理，CIC提醒，不需要为了找代理而支付费用，如果你指定了免费代理，那么使馆将会直接联系该代理人。
There are two (2) types of representatives.
Uncompensated Representatives:
Uncompensated representatives do not charge fees or receive any other form of consideration or compensation for providing advice or services to represent you before CIC or the CBSA.
Uncompensated representatives include:
- friends and family members who do not, and will not, charge a fee or receive any other consideration for their advice and services;
- organizations that do not, and will not, charge a fee or receive any other consideration for providing citizenship or immigration advice or assistance (such as a non-governmental or religious organization);
- consultants, lawyers and Quebec notaries, and students-at-law under their supervision, who do not, and will not, charge a fee or receive any other consideration to represent you.
Note: You do not have to pay someone for them to be your representative. CIC will conduct business with an uncompensated representative if an applicant appoints them on their behalf.
收费代理：如果要指定收费代理这个代理必须是 CIC 认可的，如果你提交的 5476 里的收费代理不是 CIC 认可的，那么会被直接退料。CIC 只与授权代理打交道。
Compensated representatives:
Compensated representatives charge a fee or receive some other form of consideration in exchange for the representation that they provide.
It is important to know that anyone who represents or advises you for payment - or offers to do so - in connection with CIC proceedings or applications is breaking the law unless they are an authorized representative or they have a specific agreement or arrangement with the Government of Canada that allows them to represent or advise you. This applies to advice or consultation which happens before or after a citizenship or an immigration application is made or a proceeding begins.
CIC will only conduct business with compensated representatives who are in good standing with their designated regulatory body. For more information see - Find out if your representative is authorized.
授权代理定义：ICCRC 会员，省律师协会律师，律师助理及某些法律系学员。指定收费代理必须指定这些授权代理，否则会被退料。（2026 注：ICCRC 现为 CICC，另安省 paralegal 须为 Law Society of Ontario 会员，见文首第二条。）
Authorized representatives are:
- consultants who are members in good standing of the Immigration Consultants of Canada Regulatory Council (ICCRC);
- lawyers and paralegals who are members in good standing of a Canadian provincial or territorial law society and students-at-law under their supervision;
- notaries who are members in good standing of the Chambre des notaires du Québec and students-at-law under their supervision.
If you appoint a compensated representative who is not a member in good standing of one of these designated bodies, your application will be returned.

General Application Information
Appoint a representative
- Check box to indicate if you are appointing a representative to represent you with your application process. Complete sections A, B and D.
Cancel a representative
- Check box to indicate if you are canceling a representative. Complete sections A, C and D; and
- Check both boxes and complete all sections if you are cancelling a representative and appointing a new one at the same time.
Section A – Applicant Information
Question 1 申请人姓名：Write your last name (surname or family name) and given name(s).
Question 2 申请人生日：Write your date of birth.
Question 3 递交申请的：If you have already submitted your application, write: 所递交的使馆，the name of the office where the application was submitted; and 申请类别，the type of application you have submitted.
Question 4 UCI 号：Write your Citizenship and Immigration Canada Identification (ID) or Unique Client Identifier (UCI) number (if known).
Section B – Appointment of Representative
Question 5 代理人全名，Write your representative’s full name. If your representative is a member of the ICCRC, a law society or the Chambre des notaires du Québec, print his or her name as it appears on the organization’s membership list.（2026 注：ICCRC → CICC）
Question 6 代理类别，是收费的还是免费的，Check one box to indicate if your representative is uncompensated or compensated. If your representative is compensated, write the membership ID number of: the ICCRC; or a Canadian provincial or territorial law society; or the Chambre des notaires du Québec.（2026 注：顾问填 CICC 会员号）
Question 7 代理详细信息，Write your representative’s contact information. If you are appointing a student-at-law to represent you, include their supervising lawyer’s information including their membership ID.
Note: 使馆会将邮件发到代理人邮箱，By indicating your representative’s e-mail address, you are hereby authorizing IRCC to transmit your file and personal information to this specific e-mail address.
Question 8 律师签名及签署日期，To accept responsibility for conducting business on your behalf, your representative must: sign the declaration; and date the declaration.
Section C – 取消代理 - Cancel the Appointment of a Representative
Question 9 Fill in this section if you wish to cancel the appointment of a representative. Write the representative’s full name. Complete all sections of the form if you wish to both cancel a representative and appoint a new one.
Section D – 声明 - Your Declaration
Question 10 申请人签字 By signing, you authorize IRCC to complete your request for yourself and your dependent children under 18 years of age. For sponsorship application, your spouse or common-law partner does not have to complete a separate request. If your spouse or common-law partner is included in this request, he or she must sign in the box provided.
授权向代理之外的人披露信息需要填写 5475 表 Release of information to other individuals
To authorize IRCC to release information from your case file to someone other than a representative, you will need to complete the form Authority to Release Personal Information to a Designated Individual (IMM 5475).
The person you designate under that form (IMM 5475) will be able to obtain information on your case file, such as the status of your application. However, they will not be able to conduct business on your behalf with IRCC.

- 让 AI 帮你查代理资质前先把名字拼准：把中介给你的顾问英文名、会员号、执业省份整理成一行，你再拿去 CICC 或省律师协会的在线名录里查。名录按注册全名检索，中文名音译对不上是最常见的查不到原因。
- 让 AI 核你手上的 PDF 是不是 11-2025 版：把表格首页截图丢给它，只问"右下角/页脚标注的版本日期是什么"。这一步花 10 秒，能省一次退料。
- 让 AI 把"签这张表之后谁收信"讲给家人听：很多退料与失联发生在申请人不知道信件已改发代理。让 AI 用大白话写三句话，转给出资的家人。
- AI 干不了的：不能替你判断某位顾问是否"信誉良好（in good standing）"——那是 CICC/律师协会名录上的实时状态，被停牌的人名字可能仍在网页缓存里；也不能替你签字，第 8 题与第 10 题必须本人与代理各自手签。
- 别让 AI 编会员号或代填代理信息：这张表本身就写着不实陈述属于严重犯罪，且信息会与 CICC、律师协会、RCMP 共享。
